IMPORTANT — Please Read Before Submitting

To cancel your lessons for the upcoming month, you must submit this form on or before the 1st of the month.

• Forms submitted by the 1st will cancel your membership for the upcoming month's billing.

• Forms submitted after the 1st will take effect the following month.

Examples:

• Form submitted April 18 → Membership ends May 1.

• Form submitted May 2 → Membership ends June 1.

A GN Team Member will send you a confirmation email after we process the cancellation!
